We are excited to introduce you to our new iUGO Care Principal Care Management (PCM) care program. PCM helps clinicians efficiently coordinate care for their patients with a single high-risk chronic condition.
More than 133 million Americansopens PDF file suffer from chronic illness. iUGO Care recognizes the importance of caring for this vulnerable population and has added Principal Care Management to our growing list of care management services.
Through iUGO Care, PCM helps providers monitor, track, and coordinate treatment for patients with ongoing care management focused on a single high-risk chronic condition.
iUGO Care’s PCM module allows providers to develop a disease-specific care plan, adjust medications, and make treatment decisions based on real-time patient data.
The PCM module in iUGO Care facilitates:
- Documenting treatment decisions.
- Monitoring your patient’s progress.
- Managing care transitions and referrals.
- Tracking billable hours through our PCM timer.
As an adjunct to our PCM module, our timer allows you to easily track the time you spend providing PCM services to your patients. The timer will enable you to see how much time you have spent on each patient and how much more time is needed to meet billing thresholds, ensuring you spend the right amount of time with the right patients.
The PCM timer makes it easy to bill for the Centers for Medicare and Medicaid Services billing codes HCPCS G2064 (Physician) and HCPCS G2065 (Clinical Staff), which includes 30 minutes of care management services per month.
The iUGO Care Principal Care Management care program and the timer ensures efficient patient care delivery and easy time tracking.
